Dieting vs. Liposuction — Sagging Breasts, Abdomen, and Buttocks Too
In consultations I often hear, "I lost weight by dieting, but the places I really wanted to slim down stayed the same." Some people say their face and breasts slimmed first and now look gaunt, while the thighs, buttocks, and belly will not budge. Quite a few also come in because their buttocks or breasts look even more droopy after the weight loss.
Today I will walk through how dieting and liposuction differ, starting from how fat cells work, and show with actual cases how liposuction can improve the sagging breasts, abdomen, and buttocks that are hard to resolve by dieting alone.
Dieting and Liposuction Have Different Purposes
Dieting, exercise, and liposuction all reduce body fat, so they can look alike, but their purposes are different.
A way to lose weight vs. a surgery that shapes the body
Dieting and exercise are ways to reduce body weight, while liposuction is a body-contouring surgery. Weight is managed through diet and exercise; liposuction is done to shape a beautiful body line. Many people have a normal weight but fat concentrated in specific areas, such as the outer thighs or lower abdomen, which upsets their body balance. For fat that has built up locally like this, liposuction is an effective way to reduce it. Liposuction refines the body's contour by removing excess fat through small incisions, and it is rarely performed for the purpose of losing weight[1].
| Category | Dieting / Exercise | Liposuction |
|---|---|---|
| Purpose | Weight loss and weight management | Body contouring; body line and balance |
| Change in fat cells | The number stays the same; only the size shrinks | The number of cells in the treated area decreases |
| Where fat goes first | Cannot be chosen. The face and breasts may slim first | Areas are chosen and reduced as desired |
| Sagging breasts, abdomen, buttocks | Only the volume shrinks, and the skin cannot keep up, so the area may look even more droopy | When fat is removed evenly up to the superficial layer, skin contraction is encouraged, so sagging is improved as well |
| If weight is regained | The same number of cells enlarge again | Only the remaining cells enlarge. The treated area tends to gain relatively less |
The Size and Number of Fat Cells
The difference between dieting and liposuction ultimately begins with what happens to the fat cells.
Dieting shrinks the cells; liposuction reduces their number
The number of fat cells increases until puberty, and weight gain in adulthood is mostly due to existing fat cells growing larger[2]. When you lose weight through diet or exercise, the fat inside the cells is used up and the cells simply become smaller, but their number remains. Liposuction, on the other hand, takes the fat cells themselves out of the body, so the number of cells in the treated area decreases, and fat cells removed by surgery do not grow back[2].
It is also known that keeping the same number of cells shrunken by fasting or calorie restriction is very different, in terms of appetite and hunger, from having fewer cells in the first place[3]. In addition, localized fat that does not shrink no matter how much you diet may be an area where many fat cells are clustered. Because liposuction reduces the number of cells regardless of their size, the result tends to last unless there is substantial weight gain[4].
You can reduce only the areas you want
With dieting, we cannot decide the order in which fat disappears. Usually the fat in the face goes first and the breasts get smaller, while the thighs or buttocks you wanted to slim often stay the same. Middle-aged women in particular may find that when facial fat is lost through dieting, the skin loosens and sags, making them look older and more gaunt. The biggest difference from dieting is that liposuction lets you select and reduce the fat in the areas that spoil your body line.
Areas That Are Hard to Slim by Dieting
Even when body weight goes down, some areas stubbornly stay the same. Rather than dieting harder, the aesthetically better approach for these areas is to remove only the fat that needs to go with liposuction and blend the area naturally into its surroundings. The areas we are most often asked about in consultations are as follows.
Pubic area (Y-zone)
This is the fat layer just below the lower abdomen, covering the front of the pubic bone. In many people the abdomen is flat, but only this area protrudes, so the front stands out in underwear or swimwear. After abdominal liposuction, if weight is gained, this area, where fat cells remain, may also gain relatively more. At Venus Clinic, we do not reduce the pubic area on its own; we plan it together so that the line running from the lower abdomen through the pubic area and groin to the inner thigh connects naturally. However, removing too much fat here can make it look unnatural, so we refine it carefully[10].
Under the chin (double chin)
If you have had a double chin since childhood despite a normal weight, or many family members have a similar one, it is often localized fat under the chin that does not shrink easily with dieting. People like this are good candidates for liposuction[4]. Reducing fat under the chin makes the boundary between the jawline and neckline more defined. However, a double chin caused by considerable skin laxity or by the shape of the jawbone is hard to resolve with liposuction alone, so we first sort out the cause during consultation.
Saddlebags and the inferolateral buttock
Saddlebags are the bulges on the upper part of the outer thigh, just beside the outer end of the crease between the buttock and thigh, and they make the lower body look widest from the side. This fat is pushed out to the outer thigh by fat that has built up in the lower outer part of the buttock (the inferolateral buttock), so if only the saddlebags are reduced, the tissue of the inferolateral buttock settles back down and the reduction is hardly noticeable. That is why Venus Clinic performs the Venus "Apple Line" surgery, treating the saddlebags and the inferolateral buttock together, to smooth the outline running from the waist past the hips to the thigh.
Loose upper arms
On the upper arm, fat tends to collect on the outer, inner, and back surfaces, and fat in this area is hard to remove or reduce with exercise[8]. Evenly reducing the fat under the skin of these three surfaces slims the arm, and because the weight of fat pulling the skin downward is reduced, loose skin may contract[8][9]. However, when the skin is very loose, liposuction alone has limits, so the skin condition is also assessed in consultation.
Flanks (love handles) and the lower back just above the buttocks
The flanks (love handles) are the fat that bulges out at the side of the waist above the pelvis and rests over the waistband, and the lower back just above the buttocks is the lower part of the back below the waist, right above the buttocks. Both areas change little in shape even when weight goes down, which makes the waistline look flat. Refining the flanks and the lower back together lets the waist draw inward and brings back the curve from the waist to the buttocks, and as the skin of the lower back contracts it can also pull up the top of the buttocks so they look more lifted. The fat on the upper back that folds horizontally along the bra line or just below the shoulder blades is also fibrous and does not shrink easily with dieting, so it must be suctioned thoroughly down to the layer just beneath the skin to expect a smooth back view[8]. At Venus Clinic, we refine the flanks (love handles), lower back, back, and bra line together to create a torso line that flows continuously from every angle.
Axillary fat (accessory breast tissue / armpit area)
This is the bulge of fat in front of the armpit when the arm is lowered, which shows beside a sleeveless top or a bra. The outer part of the breast and the accessory breast area have a high proportion of fat, which makes them well suited to reduction with liposuction. At Venus Clinic, we refine this as one continuous line, from the upper breast through the accessory breast and front armpit to the armpit and the flank beside the breast. Even after surgery, however, a little excess skin may show as a slight fold when the arm is held against the body.
We look at areas as connected, not separately
All of the areas above connect to their neighbors in a single line. Reducing only one area can create a boundary and look awkward, so at Venus Clinic we plan with the surrounding areas in view so they blend naturally. Results may vary by individual depending on the amount of fat and skin elasticity.
Sagging Breasts, Abdomen, and Buttocks Can Also Be Improved with Liposuction
When dieting reduces volume, the skin that wrapped that volume cannot shrink at the same speed. That is why the breasts, belly, and buttocks can look even more droopy after weight loss. Liposuction, on the other hand, does not stop at reducing volume; it can encourage the skin to contract again, so sagging breasts, a loose abdomen, and drooping buttocks can also be improved with surgery.
The worry after a successful diet — loose skin
The biggest concern for people who have succeeded at dieting is loose skin. As the fat goes away and the volume shrinks, the skin that wrapped that fat cannot shrink as much, so places that used to hold a lot of fat, such as the belly, upper arms, and inner thighs, become slack and droopy. Especially after middle age, dieting can also remove facial volume, so the skin loosens and the face may look older.
It is easy to assume that liposuction removes the fat layer between skin and muscle and makes the skin sag even more, but in practice the opposite is often true. During recovery after liposuction, the tissue beneath the skin resettles and the skin contracts, so unlike weight loss through dieting, in many cases the loose skin is drawn in along with the reduction in size. Superficial liposuction, which evenly suctions the shallow layer just beneath the skin, is the key to bringing out this skin contraction.
Ironically, however, if you have dieted hard and your skin is already loose and little fat remains, less skin contraction occurs even with liposuction. To make the skin contract enough to create a smooth line, it is actually more favorable to have liposuction while you still have a certain amount of fat. When the skin is extremely loose, liposuction alone does not pull it in sufficiently, and how much the skin will contract depends on age, genetics, and skin condition, so it is hard to know exactly in advance[12]. In particular, the lower abdomen with a large hanging fat pouch, the arms of older people or those with a lot of fat, and the inner thighs are known as areas where skin tends to remain after surgery[11].
If loose skin is your concern
In consultation, we assess the amount of remaining fat and the degree of skin laxity together. If a certain amount of fat remains and the skin sagging is up to a moderate degree, skin contraction can be expected with superficial liposuction, but if the sagging is very severe, we tell you in advance that liposuction alone has limits. Results vary by individual.
Drooping buttocks — the hidden crease becomes visible again
When the buttocks sag, the drooping buttock tissue (skin and fat) covers the original crease between the buttock and thigh. What many people think of as the crease is actually not the crease but the lower edge of the sagging buttock tissue. When buttock liposuction reduces the volume and the skin contracts, the sagging lower edge rises and sits above the crease, and the original crease that was covered becomes visible. This is the principle by which buttock ptosis is improved.
To bring out as much skin contraction as possible, Venus Clinic also performs superficial liposuction. Just below the buttock, under the crease, is where the fibrous tissue that supports the buttock runs, so only the shallow layer is carefully refined there. In addition, suctioning the hips on both sides of the upper buttock and the lower back just above the buttocks together makes the skin above the buttock contract as well, pulling the buttock upward so it lifts more. Treating the areas around the buttock together gives a much better final shape, and it has been reported that the back area, where tissue is thick and fibrous, contracts well[5].
If only the volume of the buttocks is reduced by dieting, the remaining skin often means the drooping shape does not change much. What differs from dieting is that lifting a drooping buttock requires both reducing volume and making the skin contract.
A loose abdomen — sagging of the lower abdomen can also lessen
When a lot of fat accumulates in the abdomen and the skin also loosens, the lower abdomen does not just protrude forward but droops downward and folds over the waistband or underwear line; this is called abdominal ptosis (sagging abdomen). A lower abdomen that has lost skin elasticity and developed stretch marks after childbirth or weight loss also looks loose. With dieting, only a little of the remaining fat goes and the loose skin stays as it is, so the drooping shape hardly changes.
When liposuction reduces the fat in the lower abdomen and evenly suctions the shallow layer just beneath the skin, the skin contracts during recovery, and in many cases the loose lower abdomen is drawn in and the widening of stretch marks also lessens. In a drooping abdomen, fat from above the ribs, the bra line, and the back has also come down with it, so at Venus Clinic we recommend circumferential (360°) liposuction, which refines the whole torso all the way around, not just the lower abdomen, including the flanks (love handles), lower back, and back. The contraction of sagging skin takes place gradually over about six months after surgery.
However, if there is a lot of loose skin, widespread stretch marks, or a protruding abdomen caused by separated abdominal muscles, liposuction alone may not pull it in sufficiently, so we also check the skin and abdominal wall during consultation.
Sagging breasts — when volume decreases, sagging can improve too
After childbirth or rapid dieting, even when the volume of the breast decreases, the skin that has been stretched once cannot shrink to match, so the inside empties while the skin remains and the breast droops downward. For this reason, many people think liposuction is not suitable if the breasts are sagging.
However, at Venus Clinic we regard breasts with sagging as candidates for liposuction as well. When breast volume decreases, the weight pulling downward becomes lighter, and suctioning evenly up to the superficial layer reduces the size and encourages skin contraction, so sagging is improved as well. Not only the breast but also the accessory breast, armpit, and the flank beside the breast can be refined as a single line, and because the nipple is not relocated and the glandular tissue is not cut out, no long incision scar is left and you can choose it without worrying about breastfeeding function.
Degree of improvement in sagging
How much the sagging improves depends on the original degree of sagging, skin elasticity, and the ratio of fat to glandular tissue within the breast. When skin elasticity has declined considerably or the sagging is severe, the degree of improvement may be small. Results vary by individual.
Weight Management Is Still Needed After Liposuction
Having created your body line with liposuction does not mean your lifestyle afterward no longer matters. To keep the result for a long time, please keep the following two points in mind.
If your diet and exercise stay the same, your weight can come back
The weight lost after liposuction equals the weight of the fat removed, and the body's basal energy use falls by that much as well. So if you keep your food intake and exercise exactly as before surgery, surplus calories build up, and your weight can eventually climb back close to your pre-surgery weight[6]. It is accurate to understand liposuction not as a surgery that manages your weight for you, but as creating the line with liposuction and keeping that line with weight management.
The remaining fat cells can grow again
Fat cells removed by surgery do not come back, but the remaining fat cells can enlarge again if weight is gained[2][7]. In that case, the treated area has fewer cells and tends to gain relatively less, while untreated areas may look as though they have gained more. Areas where sagging was improved, such as the buttocks and breasts, can also take on weight again and change shape if weight increases substantially, so steady weight management matters after surgery too.
Reaching the summit by elevator
Dieting is like climbing a mountain step by step: you tire out and give up before reaching the top. Liposuction is similar to riding an elevator to the summit. You can spend the willpower you saved on maintaining your weight, and your changed body line can itself become a motivation to keep up the care[3]. In fact, we see many people who, after surgery, pay more attention to weight management because they feel more attached to their figure and more interested in their health.

Before-and-After Photos
To show in real photos the point made in this article, that "sagging can also be improved with liposuction," we have prepared three cases of people who had liposuction at Venus Clinic. The areas with sagging differ in each: drooping buttocks, loose breasts, and a loose abdomen. For each person, photos taken from three directions are placed side by side with the pre-surgery photos.
Case 1 — Drooping buttocks: The tissue in the lower buttock had loosened and was folded over the thigh. Buttock liposuction reduced the volume, and by refining the shallow layer just beneath the skin we encouraged skin contraction. In the rear view, look at the lower buttock that had been folded now looking smoothed out; in the side view, the line running from the buttock to the thigh; and in the front view, the outer thigh line.
Case 2 — Loose breasts: The breasts hung down long, with the lower edge reaching toward the upper abdomen. Without cutting the skin or relocating the nipple, liposuction reduced the breast volume and encouraged skin contraction. In the front view, look at the height of the lower edge of the breast; in the 45-degree side view and the side view, look at how much the downward sagging has lessened.
Case 3 — Loose abdomen: The lower abdomen protruded forward, drooped downward, and folded over the underwear line. Abdominal liposuction reduced the fat and encouraged skin contraction to draw in the loose lower abdomen. In the front view, look at the lower abdomen that had been folded and the waist curve; in the side view, the front contour of the lower abdomen; and in the rear view, the creases on both sides of the waist.
All after-surgery photos were taken one week after surgery. Swelling and bruising are still present at this stage, so this is not the final result, and because the contraction of sagging skin proceeds gradually over several months after surgery, the shape will change a little more over time. Results may vary by individual depending on the degree of sagging and skin elasticity.
Case 1 — Drooping buttocks (rear view)
Before surgery — rear view
The loose lower buttock is folded over the thigh
1 week after surgery — rear view
The lower buttock has risen and the folded area has smoothed out
This is a case of drooping buttocks. Before surgery, the tissue in the lower buttock hung down and was folded over the thigh. This folded line, which many people think of as a crease, is actually the lower edge of the sagging buttock tissue. One week after surgery, the buttock has become smaller and its lower part has risen, and you can see that the overlapping fold has smoothed out.

In Closing
A sensible way to use both approaches is to manage weight with diet and exercise, and to refine the line of areas that dieting does not reduce, as well as sagging breasts, abdomen, and buttocks, with liposuction. At Venus Clinic, we remove the fat that needs to go as thoroughly as possible, smooth the line so that it is not bumpy, and for areas with sagging, we also encourage skin contraction with superficial liposuction.
