When women come to us for a consultation because of large breasts and I ask what bothers them, I often hear things like "My shoulders and neck always feel heavy and sore," "Nothing fits, so I only wear loose clothes," and "I'm self-conscious about people looking, so I end up hunching over." The concerns that come with large breasts are often not just about appearance; they involve physical pain, clothing, daily activities, and an emotional burden as well.

Today I would like to explain, step by step, how the breast reduction liposuction we perform at Venus Clinic reduces the breasts, how much breast volume and cup size have been reported to decrease in international studies, and why patients who have had this surgery report high satisfaction.

The Burden of Large Breasts: Body and Mind

When the breasts are large and heavy, the shoulders, neck, and back have to carry that weight all day long. Bra straps dig into the shoulders and leave grooves, the skin in the fold under the breasts can become irritated and chafed, and in trying to hide the weight, a rounded, forward-shoulder posture can become a habit. Exercise is uncomfortable because the breasts move, and it is hard to find tops that fit, which narrows the range of clothes one can wear.

Just as often as the physical discomfort, patients talk about other people's eyes. Many say that because they feel uneasy about their breasts standing out, they always choose clothes that cover the body and tend to shrink back in front of others. International studies likewise note that women with large breasts report neck, back, and shoulder pain together with limits on exercise, difficulty that leads them to avoid social situations, and trouble shopping for clothes.

How Breast Reduction Liposuction Works

The breast is made up of glandular tissue and the fat that fills the spaces in between. Breast reduction liposuction is surgery that reduces breast size by suctioning out the fat inside the breast. For this reason, the higher the proportion of fat in the breast, the better it can be reduced; if glandular tissue makes up a large share, the amount of reduction may be smaller.

Surgery Through Small Incisions

Because a thin cannula is inserted through small incisions only a few millimeters in size, no long incision lines are left around the breast or the areola. The incisions are placed in inconspicuous spots such as the armpit crease or the fold beneath the breast, and most of them fade over time. Another advantage is that no tissue is cut away, so care after surgery is not difficult.

The Mammary Glands Are Not Removed

Because liposuction removes the fat between the glands, the glandular tissue is not cut away, and there is no step of cutting out and relocating the nipple. This means women can choose it without worrying about breastfeeding function, and young women who have not yet had children and are planning marriage and childbirth can also consider breast reduction. International studies also indicate that breastfeeding function is not expected to be affected after breast reduction liposuction, and report that no decrease in nipple sensation was observed.

Not About Removing More, but About Leaving the Right Amount

Liposuction is not surgery to remove as much fat as possible; it is body contouring surgery that brings the body into balance. At Venus Clinic, rather than removing a lot, we remove an appropriate amount, considering harmony with the surrounding areas and the balance of the body as a whole. Our principle is to leave the remaining fat layer evenly beneath the skin, and to leave it consistently at the same thickness on both sides.

Including the Surrounding Areas in One Line

Women with large breasts often have fat built up not only in the breasts themselves but also in the accessory breast at the front of the armpit, in the armpit, and along the side of the chest next to the breast. If only the breasts are reduced and the surrounding areas are left as they are, the fullness around the armpit can stand out when viewed from the front or side, which can actually look unnatural.

That is why at Venus Clinic we reduce the fat in the breasts, accessory breast, armpits, and the side of the chest together, refining the line from the chest to the flank into one continuous contour. This not only helps reduce breast size effectively, but also makes the transitions to the surrounding areas look natural, so the overall shape after surgery looks natural.

Area Why it is contoured together
Breasts Fat inside the breasts is suctioned evenly to reduce their size.
Accessory breast If the bulge at the front of the armpit remains, it looks more prominent once the breasts are smaller.
Armpits The fullness around the armpit is refined so that it connects with the breast line whether the arms are lowered or raised.
Side of the chest Narrowing the width from the breast to the flank naturally connects the contours seen from the front and the side.

How Much Do the Breasts Shrink? Reports from International Studies

The question I am asked most often in consultations is "How much smaller will they get?" International liposuction textbooks and papers report the reduction in volume and cup size achieved with breast reduction liposuction as follows. These are figures reported in international papers; they do not directly represent results at Venus Clinic, and the degree of reduction varies from person to person.

What the Studies Report

① A liposuction textbook explains that tumescent liposuction can reduce the volume of a woman's breasts by up to 50%.

② In a study of 204 patients aged 15 to 74, the amount aspirated per breast was 300–2,250 cc (average 850 cc), and breasts were reported to decrease by an average of two cup sizes. The shape at 6 weeks after surgery was reportedly maintained at 6 months.

③ In a study of 45 patients aged 15 to 71 (average age 39), 350–2,250 ml (average 950 ml) was aspirated per breast, and changes in US bra size such as 40FF → 38D, 38D → 36B, 42FF → 38C, and 34D → 34C were given as examples.

These Figures Are Hard to Apply Directly to Korean Women

All of the studies above were conducted in the United States, and the women studied were mostly Western women with a large build and very large breasts. That is also why the pre-operative bra sizes in the papers are often as large as 40FF or 42FF. The amount of fat aspirated depends on the original size of the breast, so in women whose breasts are this large, more than 1,000 cc may be aspirated from one side, but it is unrealistic to expect the same aspirated volume or the same number of cup sizes of reduction in Korean women, who generally have a smaller build and smaller breasts. Even with the same surgery, it is natural for the aspirated volume to be smaller, and because bra sizing systems differ from country to country, cup sizes cannot be compared one-to-one.

So at Venus Clinic, rather than how many cc were aspirated or how many cup sizes were lost, we look at how much lighter the breasts have become relative to each person's build, and how well balanced the line from the breasts to the armpits and flanks has become. We hope you will regard the figures from international papers as evidence that liposuction alone can meaningfully reduce the breasts.

Above all, how much you want your breasts reduced is your own choice. Some women want a reduction even though their breasts are not very large, and people who use their bodies a lot, such as athletes, often want breast reduction so that their breasts do not get in the way of movement even when they are not large, and many hope to reduce them as much as possible within what is feasible. At Venus Clinic, we listen carefully during the consultation to how much reduction you want, examine the ratio of fat to glandular tissue and the condition of the skin, tell you in advance the range that can be reduced, and then plan the surgery within the principle of leaving the remaining fat layer evenly.

Things to Know in Advance

Since what liposuction can reduce is the fat inside the breast, breasts with a lot of glandular tissue may shrink less, and in some cases it may be difficult to reduce them as much as desired. Depending on skin elasticity, some sagging or wrinkling may remain. In addition, although liposuction reduces the number of fat cells, the remaining fat cells can grow again with weight gain, so if you gain weight after surgery, the breasts may grow larger as well. Pregnancy, breastfeeding, and age-related changes also affect breast size and shape. That is why we carefully assess the condition of the breasts in the pre-operative consultation and explain in advance the range of results you can expect.

Size, Sagging, and Age

In women with large breasts, the breasts usually sag because of their weight. Many people think "Sagging breasts can't be treated with liposuction," but at Venus Clinic we consider sagging breasts to be good candidates for liposuction as well. As the volume decreases, the weight pulling the breast downward becomes lighter, and when superficial liposuction, which evenly suctions fat up to the shallow layer close to the skin, is used to induce skin contraction, the size decreases while this helps improve sagging, creating a natural shape.

International studies also describe obtaining skin contraction through superficial liposuction, and report that the distance from the nipple to the hollow between the collarbones (the sternal notch) improved by an average of 6 cm. Another study reported that the nipple position rose by an average of 2.57 cm, and that in patients aged 60 and older the average was 3.16 cm, actually greater than the 1.79 cm seen in younger patients. The degree of improvement in sagging varies from person to person depending on skin elasticity and the degree of sagging.

There is a reason why size tends to decrease more with age. After middle age, the breasts often contain less glandular tissue and a relatively higher proportion of fat than those of younger women. Since what liposuction can reduce is the fat inside the breast, the more fat a breast contains after middle age, the more its size can decrease. The study above also explains that the nipples rose more in patients aged 60 and older probably because their breasts had a higher proportion of fat. Conversely, the breasts of younger women, which have more glandular tissue, may shrink relatively less.

Women Over 60 Have This Surgery, Too

At Venus Clinic, many women over the age of 60 have breast reduction liposuction. Having endured the weight of large breasts for a long time, some of them say during consultation that they wish they had known about this method earlier. Even if skin elasticity has decreased with age, there is no need to give up on surgery in advance because of age alone; we decide whether surgery is appropriate after also assessing overall health.

Why Satisfaction Is High

There is more than one reason why patients who have had breast reduction liposuction report high satisfaction. Reduced physical pain is important, but what we hear in the consultation room goes far beyond that.

The Weight on the Shoulders and Neck Becomes Lighter

Chronic pain in the shoulders, neck, and back caused by the weight of the breasts is the physical problem most commonly mentioned by women with large breasts. When breast volume decreases, the weight the shoulders and neck had been supporting is reduced, and international studies likewise report a reduction in breast-related symptoms and pain. Women who were bothered by bra strap grooves or chafing in the fold under the breasts may also find care much easier.

Freedom from Other People's Eyes

For women who used to hunch over because they felt uneasy about their breasts standing out, simply reducing breast volume can make it much easier to stand up straight and be in front of others. Not having to always choose covering clothes to avoid people's eyes is another important reason this surgery has high satisfaction.

A woman choosing a well-fitting top

You Can Choose Clothes That Fit

Many women have found it hard to choose clothes because tops that fit the chest are loose at the shoulders and waist, while tops that fit the body are tight across the chest. When the breasts, accessory breast, armpits, and the side of the chest are reduced together, the upper-body line is refined, widening the choice of fitted tops such as shirts and blouses.

Moving Your Body Becomes Enjoyable

Women who avoided running or exercise because of the weight and movement of their breasts may also find it much easier to start walking and exercising once their breasts feel lighter. When physical activity becomes comfortable, it brings energy to daily life as a whole, and this also helps maintain the body shape after surgery.

Before-and-After Photos

The three photos below show the front, 45-degree oblique, and side views of one patient who had liposuction of the breasts, accessory breast, armpits, and the side of the chest together at Venus Clinic, and all of the after photos were taken one week after surgery. The after photos still show bruising and yellowish discoloration seen during early recovery, and the swelling is still going down, so they are not the final result. The yellowish tint is part of the process of post-operative bruising fading and disappears over time.

In Closing

I believe breast reduction liposuction is not simply surgery that reduces breast size, but surgery that can help lighten the weight on the shoulders and neck, and make clothing, activity, and even the feeling of standing in front of others lighter. It is also very meaningful that, because no long incision lines are left and the glands are not removed, it can be considered by a wide range of women, from young women who have not yet had children to women over 60. At Venus Clinic, we view the breasts, accessory breast, armpits, and the side of the chest as one line, and rather than removing a lot, we evenly refine the remaining fat layer with harmony with the surrounding areas and the balance of the body in mind.

Less pain in the shoulders and neck, less self-consciousness about other people's eyes, being able to choose clothes that fit, and more comfortable exercise and activity: for these many reasons, breast reduction liposuction is a surgery with high patient satisfaction. The international study introduced above also reported a patient satisfaction rate of 93%.

We often hear this from older patients in particular. One woman in her 60s told us that she had long assumed the only way to reduce the breasts was to cut tissue away through incisions, and said she wondered why she had not known sooner about reducing them with liposuction, and that she regretted not having had the surgery earlier. Surgical results and satisfaction may vary from person to person, but if you have lived with discomfort from large breasts for a long time, I encourage you not to give up in advance because of your age and to come in for a consultation.

Note: Surgical results may vary depending on each person's body type, the ratio of fat to glandular tissue in the breasts, and skin elasticity. If there is a lot of glandular tissue, the amount of reduction may be smaller, and some sagging or wrinkling may remain, so please make your decision after a thorough consultation.
