One realistic way to reduce the buttocks is surgery that excises fat tissue and skin, but buttock reduction can also be performed relatively simply using liposuction.

Likewise, a woman's breasts can be reduced by excising fat tissue, glandular tissue, and skin all at once, but breast reduction for very large breasts is also possible using liposuction.

Some papers report that breast reduction decreases size by 2–3 cup sizes (a 5–7.5 cm decrease in chest circumference), and in terms of volume, the breasts are said to decrease by 30–50% of their original size. At Venus Clinic, we see a reduction of about 40%, and breast ptosis (sagging) of about 3–6 cm can be corrected, so satisfaction tends to be high.

Generally, people who are obese throughout the body also have more fatty tissue in the breasts, so the effect tends to be better.

Because the surgery can be performed under local or intravenous anesthesia alone, it is efficient in terms of safety and cost. Compared with excision methods, its advantages include a lower risk of losing nipple sensation, a low rate of surgical complications, and a natural rather than artificial shape after surgery. Also, because damage to the mammary glands is minimal, it is known to be less likely to interfere with breastfeeding in unmarried women.

Anatomically, a woman's breasts and buttocks have similar structures. Both are divided into deep and superficial layers of fat tissue, and both have tissue that supports (suspension) the breast or buttock so it does not droop downward, such as Cooper's ligaments in the breast and Luschka's ligament in the buttock. These maintain a fold beneath and keep the tissue from sagging, but both share the tendency to droop and flatten with age.

I believe that whether it is buttock or breast reduction using liposuction, the result should look natural after surgery, and the reduction is truly worthwhile only when the shape is lifted and rounded rather than drooping downward.

In the buttocks, however, care is needed because damage to the ligament during surgery can create a double fold under the buttocks. Another difference from breast reduction is that the distinctly feminine curve flowing smoothly into the thighs and waist must be preserved.

In practice, cases that call for buttock reduction can be divided into those where both the thighs and buttocks are large and those where only the buttocks are large.

In the former, liposuction of the thighs alone makes the buttocks look relatively larger, and surgery on the buttocks alone makes only the thighs look relatively thick, so the lower body becomes balanced only when reduction liposuction is performed on the thighs and buttocks at the same time. In the latter, where only the buttocks are large, it would be best to perform liposuction on the buttocks alone to reduce them and balance the buttocks and thighs.

The photos below are before-and-after photos of buttock reduction liposuction performed at Venus Clinic, in which reduction liposuction of the thighs and buttocks at the same time resulted in a balanced lower body. A total of 2,300 ml of pure fat was removed from the thighs and buttocks combined.

The photos below show a case of buttock reduction by liposuction of the buttocks alone. Before surgery, the buttocks were relatively large compared with the thighs; after liposuction, the sizes of the buttocks and thighs are in balance. 900 ml of pure fat was removed from the buttocks alone.
